Dear Parents & Guardians,
Our student health records are currently being updated. Please ensure you have provided an action plan if your child has one of the following:
At Sirius College we strive to provide the best care for our students. In order to maintain and update our 2017 student health records, we require all parents/guardians and your family doctor to complete an Action/Care Plan.
All Action Plans need to be completed and returned to the Sirius College Nurse by the start of term 2 Tuesday 18th April 2017. If your child’s Action Plan is not returned by this date, your child will not be permitted at school until an action plan is provided.
If your child no longer has any of the above health issues, please notify the College Nursein order to have your child’s records updated.
All Action Plans must be signed and stamped by your doctor.
